GENERAL
Do You Think You Need A Schedule Change?
Any student with a schedule problem must come to the table outside the COUNSELING OFFICE DOOR today during brunch, lunch or after-school to fill out a PROGRAM CHANGE REQUEST FORM. Valid reasons for requesting a change are:
1) You have already taken and received credit for a class.
2) You lack prerequisites for the class.
3) Lack necessary skills (attach a note from teacher documenting this)
4) Adding a class for credit (juniors & seniors) (teacher signature required to add a student service)
5) Have fewer than 5 classes (fewer than 6 classes for 9th and 10th graders)
NOTE: After Athletic Teams hold try-outs, COACHES will give Counselors lists of students to add to Canyon's sports teams, and schedules will be changed at that time. Please do not request to be added to an athletic team unless your coach has given you a signed note to do so.
DO NOT FILL OUT THE FORM TO REQUEST TEACHER CHANGES, BRUNCH CHANGES, or PERIOD CHANGES.
Please fill out ONLY ONE form and allow Counselors four school days to respond to your request with either a new schedule or your form returned to you with reason of denial. You MUST attend ALL classes on your current schedule until you receive a NEW schedule. The Program Change Request Form is Canyon's procedure for requesting valid schedule changes. Completed forms should be placed in the PROGRAM CHANGE BASKET located outside the Counseling Office. We thank you for following these registration procedures.
