OFFICIAL TRANSCRIPTS:
 
ALL REQUESTS are processed in 24 HOURS

Hours from 7:30 am to 12:00 pm
Monday through Thursday
Fridays by appointment ONLY
 
 
All requests for OFFICIAL Transcripts must be submitted in writing either by mailing your request or coming in to the Registrar’s Office and filling out our form.
 
There is a 24 hour turn around time and a $3.00 charge for the first transcript and $1.00 for each additional transcript when ordered at the same time for former students or former graduates.  Checks can be made payable to Canyon HS ASB. 
 
The following information is needed when you are requesting your transcript:
 
Name while attending Canyon High School
 
Date of Birth
 
Graduation Year
 
Your Signature
 
Address where you need to have transcript mailed to
 
Legible copy of your photo ID, such as a driver's license (if mailing the request)
Phone and/or cell phone number to reach you in case there are any questions
